Resources for Health Care Providers

The following provider tools have been compiled from a variety of resources and are designed to facilitate the management of patients with asthma.

  1.     Strategic Plan for Asthma in California (2008-12)
         
The California Department of Public Health’s Strategic Plan for Asthma in California, 2008-2012 builds
          upon considerable work over the last five years to implement the 2002 Plan. The plan provides both
          innovative and proven strategies to improve the health and quality of life of the over five million
          Californians affected by asthma. It is designed to guide collaborative action by state agencies, health
          organizations, and members of the community to effectively address asthma in California.


  2.    CAPHI's Summary of the NAEPP's EPR-3: Guidelines for the Diagnosis and Management of Asthma
         CAPHI produced this 2-page summary of NAEPP EPR-3: Guidelines for the Diagnosis and
         Management of Asthma. Please contact CAPHI (patricia.laija@cdph.ca.gov) to request laminated 
         copies.

  3.    NAEPP EPR-3: Guidelines for the Diagnosis and Management of Asthma
         Expert Panel Report (EPR-3): Guidelines for the Diagnosis and Management of Asthma;  Full Report
         from the National Asthma Education and Prevention Program (NAEPP) of the National Heart, Lung, and
         Blood Institute (NHLBI)  Released 2007.


  4.    NAEPP EPR-3: Guidelines for the Diagnosis and Management of Asthma Summary Report 2007
         Summary report of NAEPP EPR-3: Guidelines for the Diagnosis and Management of Asthma.

  5.    NAEPP Guidelines Implementation Panel Report for EPR-3: Guidelines for the Diagnosis and Management of Asthma
         Panel report from NAEPP on putting the EPR-3 Guidelines into practice.

  6.    NEEF Environmental History Form for Pediatric Asthma Patient (English/Spanish)
         Developed by the National Environmental Education Foundation, this form is intended for use with
       children already diagnosed with asthma. Designed for ease of use, the Environmental History 
       Form is not comprehensive and is intended as an initial intake tool.  


  7.   NEEF Pediatric Environmental History Form (English/Spanish)
        Developed by the National Environmental Education Foundation, this screening environmental
      history is designed to capture most of the common environmental exposures to children.  It can
      be administered regularly during well-child visits as well as to assess whether an environmental
      exposure plays a role in a child’s symptoms.

 
  8.   Asthma Control Test (ACT)
        The ACT is a simple, 5-question tool that is self-administered by the patient.  It is recognized by the 
        National Institutes of Health and is clinically validated by specialist assessment and spirometry.

9.    Asthma Therapy Assessment Questionnaire (ATAQ)
       The ATAQ instrument for children and adolescents is designed for patients aged 5 to 17 years and can
       be completed by a parent or other designated caregiver.

 

10.   CAPHI's Asthma Visit Flow Sheet is an example of an NAEPP EPR-3 based flow sheet designed to
         support the assessment and documentation of asthma care processes at clinic visits for individual
         asthma patients over time. It is composed of the four components of asthma care EPR-3 guidelines
         deem necessary to be addressed at every asthma visit.

 

         The CAPHI Asthma Flow Sheet Instructions provides an introduction to the use of the CAPHI Asthma
         Visit Flow Sheet for providers who may be unfamiliar with the EPR-3 guidelines or the use of an 
         asthma flow sheet.


11.  HFA Transition Physician Brochure  
       AANMA guide for physicians to assist their patients with the transition from MDIs that contain
     CFC propellants to MDIs that contain HFA propellants.
